Position Description

Job Title: Intensive Care Coordinator (RN Case Manager)

Key Responsibilities and Activities

  • Complex Clinical Care Coordination: Responsible for members with the highest acuity/risk.
  • Comprehensive Assessments: Conducting thorough, biopsychosocial assessments to evaluate clinical history, support, and complex needs (physical health, behavioral health, co-occurring disorders, social determinants of health).
  • Care Planning: Developing, implementing, and monitoring strengths-based care plans
  • Coordination and Intervention: Coordinating services, monitoring providers, delivering care coordination interventions via multiple modalities (mobile, in-home, online, telephone), and liaising between providers, family, and the Utilization Management teams.
  • Risk Reduction: Identifying risk factors leading to increased utilization and working to reduce preventable hospitalizations, readmissions, and inappropriate utilization.
  • Documentation: Completing all documentation in the expected time frame, with a strong emphasis on detail

Qualifications

  1. RN License in Colorado.
  2. Must live in the Denver Metro area.
  3. Clinical experience with patients/people with complex medical conditions.
  4. Critical thinking skills and the ability to draw conclusions based on assessment.
  5. Thorough documentation and detail-oriented skills.

Experience and Education

  • Experience: At least three years of clinical, care management, and/or healthcare experience, with a strong preference for hospital experience
  • Education: RN License in Colorado.

Schedule: Monday-Friday 8-5pm

  • This is 95% a work from home position with very occasional visits to hospitals or health centers if needed
